Family refuse to blame doctor after father’s death

She said measures had been taken to improve training for junior doctors.

Recording a narrative verdict coroner Roger Whittaker praised Dr Tesh for his honesty and for accepting responsibility.

He added: “I’m satisfied there is no systemic failure in all this.”

Andrew Pearson said the inquest had answered the family’s questions.

He added: “It’s allowed us to find out exactly what happened.

“We are content the coroner found there had been no systemic failure and we are happy that the trust will do all in their power to minimise the risk of any similar incident happening again.

“We would like to thank the doctors for their open and honest approach.”
